What kind of personal information is collected and stored?

Personal information is information or an opinion that identifies an individual. Akeso Massage Therapy may collect the following personal information from you: name, date of birth, email address, phone number, home address, emergency contact name and phone number, details of health conditions and wellbeing, both past and present, records of your visits to Akeso Massage Therapy including details of your health and wellbeing on the day, the treatment given and how you responded. Details relevant to COVID-19 may also be collected – information about travel, locations visited, symptoms of illness, details about COVID-19 tests. Akeso Massage Therapy aims to collect only information necessary to the provision of our services.

How is personal information collected and where is it stored?

Personal information is collected directly from you or from the person acting on your behalf (for example a parent/guardian on behalf of a child/minor). Personal information is collected via the contact form on the website, via SMS/text messages, phone calls, emails and forms such as intake form and COVID-19 screening form and verbally, in consultation. These details are stored on paper in client files kept in a secure cabinet. Some details are also stored on a secure computer, in a mobile phone and in an emailing service.

Personal information is collected primarily for the purpose of providing our services. The information collected allows Akeso Massage Therapy to contact you, to keep you informed about Akeso Massage Therapy and the service you receive and to provide you with a safe and effective service. Wherever possible Akeso Massage Therapy will ensure you are providing information with your consent and with an understanding of the reasons for the information being collected. Your information will not be given to other parties, except where there is a legal requirement. Akeso Massage Therapy will protect your personal information by reasonable safeguards against loss or theft, unauthorised access, disclosure, copying or modification.

You can request to access your personal information or request corrrections to your personal information at any time by contacting Akeso Massage Therapy. 

How long do we keep your personal information?

Information is stored as safely as possible for the period required under the Health Records Act 2001 (seven years for an adult). When no longer required the information is disposed of in a safe manner, with your privacy of our utmost concern. 

How to lodge a complaint?

If you think your information has been mishandled you can contact Akeso Massage Therapy. The complaint will be investigated and you will be advised as to the outcome. If you are dissatisfied with the outcome you can refer your complaint to the Office of the Australian Information Commissioner.

You can also request that we erase any personal data we hold about you. This does not include any data we are obliged to keep for administrative, legal, or security purposes.

Visitor comments may be checked through an automated spam detection service. Viewer data might be sent to our analytics services. This will be anonymous data.
